3 on 6 vs All-on-6 Dental Implants: Which Is Better?

Patients researching full-mouth implant solutions often come across two terms that sound similar but are quite different: 3 on 6 and All-on-6. Both use six implants per arch. Both aim to replace a full set of teeth. Yet the design philosophy behind each approach leads to meaningful differences in reliability, maintenance, and long-term performance.

At Smiles Peru we regularly evaluate both concepts when planning full-arch cases. Below is a clear, clinical comparison so you can understand the real distinctions — and why we generally recommend a properly designed All-on-6 solution for most patients.

What Is the 3 on 6 Concept?

The 3 on 6 technique places six dental implants in one arch to support three separate fixed bridges (typically two posterior and one anterior). Each bridge is usually supported by only two implants.

Marketing for this approach often emphasizes:

  • No bulky artificial gums
  • Preservation of natural bone and gum tissue
  • A more “segmented” feel similar to natural teeth

These claims can be attractive, especially to patients who want to avoid anything that looks or feels like a denture.

What Is All-on-6?

All-on-6 places six implants to support a single, continuous fixed prosthesis (hybrid or zirconia). The six implants work together as a unified system to distribute biting forces across the entire arch.

When bone volume and quality allow, All-on-6 is one of the most predictable full-arch solutions available today. It offers excellent stability, good long-term data, and the ability to design highly natural-looking restorations.

3 on 6 Compared to All-on-6

Feature 3 on 6 All-on-6
Number of implants 6 per arch 6 per arch
Prosthesis design 3 separate bridges One continuous prosthesis
Support per section Usually 2 implants per bridge All 6 implants share the load
Artificial gum (pink) Usually none or minimal Can be minimal or more, depending on anatomy
Bone reduction Often marketed as “none” Only when clinically necessary
Force distribution Segmented Continuous / shared
Repair if one implant fails That entire bridge section is compromised Remaining implants still support the arch
Hygiene access Gaps between bridges Designed for cleanability under the prosthesis
Long-term clinical track record Newer, more limited published data Extensive experience and studies

The Critical Difference: Two Implants vs. Six Working Together

This is the most important clinical distinction.

In the classic 3 on 6 design, each bridge relies on only two implants. If one of those two implants develops a problem (loss of integration, fracture, or peri-implant issues), that entire section of the restoration can fail. The other two bridges remain, but the patient loses function and aesthetics in one segment and may need significant corrective treatment.

In a well-planned All-on-6, the six implants function as a single system. Forces are shared. If one implant ever needs attention, the remaining five still support the prosthesis. This redundancy is a meaningful advantage for long-term reliability.

About Bone Removal and “False Gums”

Many patients arrive with the belief that All-on-4 or All-on-6 always requires aggressive bone removal and thick artificial gums. This is a common oversimplification.

In reality:

  • Modest bone contouring is performed only when it improves the final result or creates a stable platform after extractions.
  • When a patient has good remaining bone, reduction can be minimal or avoided.
  • The amount of pink porcelain (or acrylic) is determined by the patient’s anatomy and smile line — not by the number of implants.
  • In suitable cases we can deliver porcelain or zirconia restorations with only minimal pink coloring limited to the interdental papilla area.

We place implants where the best quality and quantity of bone exists. We do not force implants into predetermined positions simply to fit a fixed prosthetic design. This biologically driven approach is generally more predictable than a strictly prosthetically driven protocol.

When Might 3 on 6 Be Considered?

A segmented three-bridge design can make sense in carefully selected cases with excellent, evenly distributed bone and ideal anatomy. However, for the majority of patients seeking full-arch rehabilitation — especially those with some degree of bone loss or complex bite forces — the continuous All-on-6 design offers better biomechanical safety and simpler long-term maintenance.

Our Recommendation at Smiles Peru

We evaluate every case individually with 3D imaging. When bone volume allows, we generally prefer a well-planned All-on-6 because:

  • Six implants share the load across the entire arch
  • The design has greater redundancy if one implant ever needs attention
  • Implant positions can still be chosen according to the best available bone
  • The final prosthesis can be designed with minimal or no bulky pink material when anatomy permits
  • Long-term clinical experience with continuous full-arch prostheses is extensive

Most of our full-arch patients receive a hybrid prosthesis because they have already experienced significant bone loss. When a patient presents with good bone volume, we can often achieve a more natural-looking result using porcelain or zirconia with only minimal pink coloring.

Next Step

If you are researching 3 on 6 or All-on-6 solutions, the most reliable next step is a proper evaluation with 3D imaging. Send us your existing X-rays or CBCT (or we can arrange new imaging). We will review your bone volume, anatomy, and goals and provide a clear recommendation tailored to your case — not to a marketing label.
